Sagging skin and wrinkles are part of aging. Skin aging is both intrinsic and extrinsic. Intrinsic skin aging is determined by genes, in women also menopause (changing hormones) and slower metabolism. Because we produce less collagen and elastin fibers, the skin becomes less supple. The older body loses bone tissue, subcutaneous fat and muscle mass, making you appear older, especially in the face. Extrinsic skin aging is caused by external influences such as smoking, alcohol, stress and exposure to sunlight. Avoiding these as much as possible keeps your skin in good condition longer.
Our body builds bone tissue at a slower rate, at the same time its quantity decreases. Our head becomes smaller due to the shrinking of the skull. The subcutaneous fat in the face sinks down. Consequently, the skin becomes too large for the shrinking head, resulting in wrinkles and hollow eye sockets. Loss of bone tissue and subcutaneous fat in the upper jaw causes a flat lip/nose crease and drooping corners of the mouth. The lips partially fall inward, making them appear thinner.
If you are young then the subcutaneous fat in your face is evenly distributed. In older people this distribution is less favorable because subcutaneous fat partially disappears and the remaining part sinks down. The temples become hollow, cheekbones less prominent and puffiness develops around the eyes.
In addition to bone loss (shrinking of the skeleton and skull), there is also volume loss, a different fat distribution (fat sinks to the bottom) and reduction of deep subcutaneous fat (you also lose when you get slimmer). As the subcutaneous fat decreases, you lose skin support. As a result, the void of skin becomes more and more visible and therefore the structure of the skin changes. In fact, deep subcutaneous fat is essential for smooth skin and facial elevator. When that is missing, you get a tighter appearance and any change due to aging is also more visible. In addition, the skin becomes thinner, dry (because the skin produces less sebum) and less supple. The face becomes smaller causing the skin to wrinkle.
After menopause, the skin thins up to 30%, which means that it needs more support from the subcutaneous fat. As a result, you lose support in your face which also changes the shape of your face.

Her cheeks are more lifted which also results in the elevator of the corners of her mouth. She looks more cheerful and friendly again.
